I am also in-network with several EAP plans including Lyra, Modern Health and Optum.

Haxby Counseling is currently not in-network with any Medicaid or Medicare plans including supplemental plans.  

In the event that Haxby Counseling is an in-network provider for your insurance carrier, it is your responsibility to know your mental health benefits. 

Acceptable forms of payment include cash and debit/credit cards including HSA and FSA plans.
